Transportation Safety Most Wanted

  1. DOT: Amend 49 CFR 1131 General Authority Mission Statement for the NTSB to to add the (s) behind cause and probable cause. Eliminate the mandate to determine a singular cause. Doing so would be educational to the public we support, increasing their awareness in a systems approach to safety, and move the safety culture to one of continuous improvement. The NTSB already does look for all material and human factors during mishap investigations, so the CFR should be amended to reflect that. DOT:
  2. DOT: Eliminate DOT On Time Performance metric for airlines. On time metrics can create artificial time pressure, which is a hazard. The best mitigation is to eliminate it at the Federal Level.
  3. FAA Provide safety education and and require experience commensurate level of responsibility for Directors of Safety, Safety Managers and Safety Practitioners.
  4. FRA require placards also be placed on underside of train cars carrying hazardous materials, in addition to sides and ends.
  5. DOT: Enable open access to de-identified safety reporting system databases for all contributors to that system, examples the Aviation Safety Action Program (ASAP) and Aviation Safety Information Analysis and Sharing (ASIAS).
